• Healthcare Provider Enrollment Requests – You will be given several copies of this form that you can give to doctors that are not members of SimpleCare so that they can sign up and you can both begin enjoying the SimpleCare benefits.

• Access to our Wellness Website – You will be given a username/password so you can log in to our Wellness Website – www.simplecare.meta-ehealth.com. Here you’ll find a wide variety of articles, tips, and contacts that can help you stay healthy without having to go to the doctor. Wellness is the best way to save on medical expenses!

• Catastrophic Health Insurance Information – We’ll provide you giudance to get catastrophic health insurance for you, your family, or your employees. Such insurance covers major medical costs and is considerably cheaper than normal health insurance.
